Job Description
Population Health Navigators demonstrate outstanding customer service skills and use evolving technology to manage a wide range of patient needs and requests, such as appointment scheduling, connecting others with resources, navigating the Cleveland Clinic Health System network, following up with general and/or health-related questions and navigating insurance benefits and coverage. Caregivers in this role are also responsible for coordinating population health activities and assisting with coordinating care, processes, activities and duties of the Ambulatory Care Management, Community Care and Quality Alliance departments.
**Remote based in Florida only**A caregiver in this position works 8:00AM – 5:00PM or 8:00am – 4:30pm
A caregiver who excels in this role will:
Collaborate with caregivers across the continuum to assist in patient navigation for low, moderate and rising risk patients.
Perform activities to proactively close care gaps, assist with health maintenance, schedule office and preventative care visits, and assist with navigating health insurance benefits and coverage. Outreach can occur via telephone, letter, or MyChart, amongst others.
Perform data gap closure in the payor portals.
Assist with coordinating population health activities to help improve quality and safety outcomes for an assigned patient population.
Assist with PCP fidelity.
Perform onboarding and off-boarding activities to assist patients with seamless provider-related transitions.
Facilitate care and connection to Cleveland Clinic services with benefit alignment.
Pend orders for PCP and schedule follow up appointments.
Collaborate effectively with all members of the primary care team, leadership, and multi-disciplinary caregivers.
Support and participate in process improvement efforts.
Serve as a liaison between the navigation and primary care team members, insurance companies, and patients.
Attend teamlet meetings for assigned payors.
Provide coaching and support to the Population Health Navigator I team members for workflow questions.
Review clinical documentation and coding to determine that conditions are actively being managed including staff messages to providers.
Provide coverage to peers.
Participate in educating patients about Cleveland Clinic services and network navigation, promoting available programs and services.
Minimum qualifications for the ideal future caregiver include:
High school diploma or GED.
Three years of recent experience working with patients in a clinic setting or an equivalent position that interacts with patients, physicians and other departments.
Knowledge of data collection, analysis, medical terminology and HIPAA regulations.
Proficient in Microsoft Office.
Preferred qualifications for the ideal future caregiver include:
Associate’s or bachelor’s degree. This will substitute one year of the required experience.

Physical Requirements:
Must be able to sit for prolonged periods of time.
Possess good visual acuity through normal or corrected vision.
Possess dexterity to operate a PC and other office equipment.
Light driving to outside locations within the region.
Personal Protective Equipment:
Follow standard precautions using personal protective equipment as required.
