The Clinical Outcomes Specialist audits records and analyzes selected clinical data to evaluate clinical outcomes and regulatory compliance.

This is a remote position. A caregiver in this role works Monday through Friday from 8:00am to 5:00pm. No weekend or holiday requirements.

A caregiver who excels in this role will:

  • Perform chart audits, event outcomes audits and record review to ensure compliance with regulatory requirements and to track/monitor/trend results of performance improvement initiatives.
  • Assist managers with identifying performance problems related to documentation, care planning and OASIS compliance.
  • Participate in performance improvement initiatives and act as a resource for managers in evaluating staff competencies.
  • Provide coaching and remedial instruction and training.
  • Review outcomes data and identify problems, trends and opportunities for improvement during record review.
  • Report data to internal customers as required.

Minimum qualifications for the ideal future caregiver include:

  • Graduate of an accredited school of professional nursing
  • Current licensure as a Registered Nurse (RN) in the State of Ohio
  • Minimum two years of nursing experience

Preferred qualifications for the ideal future caregiver include:

  • Bachelor's of Science in Nursing
  • Home health experience when supporting the home care practice or hospice experience when supporting the hospice program

Physical Requirements:

  • Ability to sit or stand for extended periods of time.
  • Must be able to climb stairs, pull, push and facilitate movement of the patient.
  • Must have normal or correctable vision.
  • Must have normal or correctable hearing to be able to hear normal conversations.

Personal Protective Equipment:

  • Follows standard precautions using personal protective equipment as required.

Pay Range

Minimum Annual Salary: $63,250.00

Maximum Annual Salary: $96,467.50

The pay range displayed on this job posting reflects the anticipated range for new hires. A successful candidate’s actual compensation will be determined after taking factors into consideration such as the candidate’s work history, experience, skill set and education. The pay range displayed does not include any applicable pay practices (e.g., shift differentials, overtime, etc.).
